This will be a little different because the concentration of motion is on the base of the penis rather than the top. You will need to grip the head of the penis with one hand while stroking the bottom of the shaft with the other hand. As you pull down on the testicles (balls), you can feel the head of  the penis move down a little bit in the other hand. When you get used to the motion on this technique, you can experiment with squeezing or turning the hand you're using to hold the head.
